Member Enrollment Form – Through our partnership with VeracityRx, members have access to receive their high-cost brand and Specialty medications through alternative programs.  Members will need to complete the “enrollment form” to get started.

Note – Enrollment is only needed for members on brand and Specialty medications.

Member Portal

Members have access to their pharmacy benefits through STAR-Rx.  Through the portal you can:

  • Refill of prescription
  • Check benefit information
  • Price a drug
  • Find a pharmacy
  • Print a temporary ID card
  • View your pharmacy history
  • Print or save you prescription summary report
  • Register for mail order
  • Much more

To register as a new user, you will need a copy of your insurance card.

In the registration form, enter your:

  • Bin # (018364)
  • Card ID (can be found on your medical insurance card)
  • Date of birth
  • Desired user name (at least six (6) characters)
  • Email address
  • Gender
  • Verification questions
  • Verification answers
